1. WHAT IS TRAVEL INSURANCE?

Travel Insurance is designed to provide medical protection, protection for your belongings and cost of your trip.


2. WHAT IS PATRIOT TRAVEL INSUANCE?

Patriot Travel Insurance can be purchased through hbs \Travel Insurance, and is underwritten by International Medical Group. There are two Patriot Travel Plans. Patriot International provides coverage for US Citizens travelling outside the US with coverage for brief returns to the US – while Patriot America provides converge for non US citizens travelling outside their home country. Both plans are available for a minimum of 10 days up to a maximum of two (2) years.


3. HOW DO I PURCHASE PATRIOT TRAVEL INSURANCE?

Simply log on to hbstravelinsurance.com – click on to get a quote, and select the plan that best suits your needs. Our website and that of our Underwriter IMG have state of the art security, so you don’t have to worry about providing your personal details.


4. HOW DO I GET MY TRAVEL INSURANCE POLICY DOCUMENT?

Your policy document will be sent to you by e-mail as soon as your payment details have been validated. A copy of your Travel Insurance Policy wording is also available for you to review by simply clicking on the appropriate Policy Document icon.


5. WHO DO I CONTACT IN EVENT OF A QUERY ABOUT MY INSURANCE?

6. DO I NEED TO CARRY A COPY OF MY TRAVEL INSURANCE DOCUMENT WITH           ME WHEN TRAVELLING?

Although you do not need to carry your Travel Insurance Policy Document with you for your cover to be in effect, we recommend that you take it with you every time you travel. It is an important document, among other things it provides you with details of what to do in the event of a medical emergency.


7. WHEN DOES MY COVERAGE BEGIN? WHEN DOES IT END?

Coverage begins:

(a) The date your application and premium are received by IMG.
(b) The date coverage is requested on your application
(c) The date you depart from your home country

Coverage ends:

(a) The end of the period for which premium has been paid
(b) The date requested on your application form
(c) The date you return to your home country - However for every 5 months of continued coverage you purchase you can purchase one additional month of home coverage as an accommodation and supplemental travel benefit, up to a maximum of 2 months.


8. ARE THERE EXCLUSIONS AND LIMITATION TO MY POLICY?

Yes: Our Travel Insurance has a number of exclusions and limitations. For example – Pre-existing condition has limited coverage. For a complete listing of exclusions and limitations, please refer to your Policy Document.


9. I HAVE A MEDICAL CONDITION CAN I BUY YOUR TRAVEL INSURANCE?

Yes: you can still buy our travel insurance, but it is important to note that you may not be covered for some Pre-existing Conditions or have limited coverage for some existing illness or medical conditions. Please ensure you read the terms of the pre-existing clause of your policy carefully.


10 WHAT IS MEANT BY PRE-EXISITING CONDITION?

Pre-existing Condition refers to injury, illness, sickness, disease or other medical condition that you know about or should know about or existed before you bought this insurance. For a complete description of Pre-existing Condition please refer to the general exclusion on your Travel Insurance Policy.


11. WHAT IF I CHANGE MY MIND AFTER I PURCHASE YOUR TRAVEL                                    INSURANCE?

Your satisfaction is very import to us. If you are not pleased with our product for any reason, you may submit a written request, prior to the effective date for cancellation and refund of your premium. If you do not have any claim filed with IMG you may cancel your plan after the effective date; however the following conditions will apply.

(i) You will be required to pay a US$50 cancellation fee

(ii) Only full month premiums will be considered for refunds. (e.g. If you choose to cancel

your coverage two months and two weeks prior to the date your coverage ends, IMG will only consider the two full months for a refund. If you have filed a claim your premium is not refundable.


12. DO AGE RESTRICTION APPLY?

Yes. Persons 80 and over can purchase our Travel Insurance but have limited coverage.


13. CAN I INSURE MY PARTNER?

Yes. You can insurance your partner as well as your children.


14. MUST CHILDREN BE ACCOMPANIED BY THEIR PARENTS IN ORDER TO                     QUALIFY FOR TRAVEL INSUANCE?

No.


15. DOES MY TRAVEL INSURANCE COVER ME IF I AM TRAVELLING ON                              BUSINESS?

Yes: You are fully covered for business travel.



16. WHAT SERVICE DOES IMG OFFER?

My IMG is our online service that allows you to access information and manage accounts 24 hours a day, seven days a week 365 days a year, from anywhere in the world, our Service Centre in the USA and Europe are always available to help or handle emergencies 24 hours a day, seven days a week, 365 days a year, from anywhere in the world. Through my IMG you have immediate access to a wealth of information about your account and can manage routine areas to help you save time when you need it most. Some features include:

(i) Get Explanation of Benefits
(ii) Locate a Provider
(iii) Request ID Cards
(iv) Initiate Pre-certification
(v) Obtain Certificate Documents
(vi) Recommend Provider/Facility

17. WHO DO I CONTACT IN CASE OF A MEDICAL EMERGENCY?

In the event of a Medical Emergency you must contact IMG – in the USA 1(800) 624-4664 (toll-free) or 1(317) 655-4500 call IMG outside the USA: 001(317)655-4500 (collect if necessary). This information is also provided on your ID Card.


18. HOW DO I MAKE A CLAIM?

Any occurrence accidents, illness or loss which may give rise to a claim should be reported to IMG –E-mail: insurance@imglobal.com or 1(317) 655-4500 or www.imglobal.com. Please read your Travel Policy Document re Claims Procedure.


19. AM I COVERED FOR SPORTS AND ACTIVITIES?

Each Patriot Policy covers injuries incurred during amateur athletic activities which are non-contact and engaged in solely for leisure, recreation, entertainment or fitness purposes. Some of these sports and activities include but not limited the motorcycle/motor scooter riding, scuba diving (10m) snorkeling, wake boarding and water skiing. Please read your Travel Insurance Document for more details. You can purchase the Adventure Sports Rider for more benefits.


20. AM I COVERED IF I AM PREGNANT?

Yes: You can purchase our Travel Insurance if you are pregnant; however the pregnancy and illness relating to the pregnancy will not be covered.


21. ARE THERE COUNTRIES EX CLUDED FROM MY TRAVEL INSURANCE?

Coverage is open to most countries, however for some benefits including evacuation and terrorism will not be covered in some countries. Please read your Travel Insurance Policy Document for further details.


22. WHAT DOCUMENTS OR INFORMATION DO I NEED TO HAVE WHEN I AM                       APPLYING FOR PATRIOT TRAVEL INSURANCE?

You will need the following:

(1) Passport Information
(2) Date of Travel
(3) Intended Destination
(4) Credit card details if you are paying by credit card
